entertainment Steroids in severe community-acquired pneumonia By breathe.ersjournals.com Published On :: 2024-10-01T10:05:24-07:00 There is conflicting evidence regarding the use of steroids in severe community-acquired pneumonia (CAP), with previous randomised controlled trials limited by small sample sizes. ESCAPe and CAPE COD are two recently published large trials on steroids in severe CAP. ESCAPe assessed the initiation of methylprednisolone within 72–96 h of hospital admission, while CAPE COD studied the use of hydrocortisone within 24 h of the development of severe CAP. ESCAPe did not show any differences in all-cause 60-day mortality or any of its secondary outcomes. CAPE COD showed that hydrocortisone improved all-cause 28-day mortality and reduced the risk of intubation or vasopressor-dependent shock. Important differences between the trials included the steroid regimens used, timing of steroid administration and baseline characteristics, with more diabetic patients included in ESCAPe. The results of CAPE COD support the initiation of hydrocortisone within 24 h of developing severe CAP, but more research is needed to evaluate long-term outcomes and optimum dosing regimens for steroids in severe CAP. Recent advancements in minimally invasive techniques, molecular biology and novel drug discoveries have accelerated the refinement of stage III NSCLC management. The latest developments in staging include the forthcoming update of the nodal component in the 9th TNM (tumour–node–metastasis) edition, which emphasises the critical role for endobronchial ultrasonography in mediastinal staging. Recent treatment developments include the use of immunotherapy and targeted molecular therapy in both the neoadjuvant and adjuvant setting, either in combination with other modalities or used alone as consolidation. Surgical and radiotherapy advancements have further enhanced patient outcomes. These developments have significantly improved the prognosis for patients with stage III NSCLC. Fast-changing recommendations have also brought about a challenge, with clinicians facing a number of options to choose from. Therefore, a multimodal approach by a multidisciplinary team has become even more crucial in managing stage III NSCLC. Full Article

It has proven invaluable as therapy for haematological and immunological disorders, and more recently in the treatment of metabolic and enzyme disorders. As the number of performed transplants grows annually, and with patients enjoying improved survival, a knowledge of both early and late complications of HSCT is essential for respiratory trainees and physicians in practice. This article highlights the spectrum of respiratory complications, both infectious and non-infectious, the timeline of their likely occurrence, and the approaches used for diagnosis and treatment, keeping in mind that more than one entity may occur simultaneously. As respiratory issues are often a leading cause of short- and long-term morbidity, consideration of a combined haematology/respiratory clinic may prove useful in this patient population. Full Article

In recent years, IPCs have become popular due to the presumed convenience of an outpatient procedure followed by home drainage leading to a misconception of IPCs being an ideal treatment for MPE. However, IPCs predispose the patient to multiple complications and have significant physical and psychological implications that are under-recognised. Patients require additional clinical reviews, hospital admissions and treatment for these complications related to IPCs. Additionally, there is a huge psychological impact of living with a home catheter that is a constant reminder of their cancer and this has been shown to affect quality of life negatively. Hence, IPCs should not be considered the "ideal" treatment for MPE management and clinicians should reflect the equipoise of the evidence for the benefits and accurately reflect the adverse effects of IPCs in their discussions with patients to facilitate informed decision making. The patients with the most severe forms of pulmonary infection may not be surgical candidates due to their underlying pulmonary condition. The management of haemoptysis can include tranexamic acid, bronchial artery embolisation, antifungals or surgery. The presence of type 2 (T2) inflammation is a well-recognised feature of asthma; however, it is now apparent that a subset of COPD patients also displays evidence of T2 inflammation with respect to elevated eosinophil counts and altered gene and protein expression of several T2 inflammatory mediators. T2 inflammatory mediators represent an attractive therapeutic target in both COPD and asthma; however, the efficacy of pharmaceutical interventions varies between diseases. Furthermore, the nature of some shared clinical features also differs. We provide a narrative review of differences in the nature of T2 inflammation between COPD and asthma, which may partly explain phenotypic differences between diseases. We focus on evidence from studies of pulmonary histopathology, sputum and epithelial gene and protein expression, and response to pharmacological interventions targeted at T2 inflammation. However, there is extensive ongoing research in this fast-evolving field. Recent improvements in spatial imaging have allowed us to study the three-dimensional structure of the lung, providing both spatial architecture and transcriptomic information at single-cell resolution. This fast progression, however, comes with several challenges, including significant image file storage and network capacity issues, increased costs, data processing and analysis, the role of artificial intelligence and machine learning, and mechanisms to combine several modalities. It is an interprofessional specialty, primarily aiming to improve quality of care for cancer patients and their families, from the time of diagnosis of malignant disease, over the continuum of cancer care, and extending after the patient's death to the period of bereavement to support the patient's family. There are various complex and frequently unmet needs of lung cancer patients and their families/caregivers, not only physical but also psychological, social, spiritual and cultural. Systematic monitoring of patients’ symptoms using validated questionnaires and patient-reported outcomes (PROs), on a regular basis, is highly encouraged and recommended in recent guidelines on the role of PRO measures in the continuum of cancer clinical care. It improves patient–physician communication, physician awareness of symptoms, symptom control, patient satisfaction, health-related quality of life and cost-effectiveness. This implies that all treating physicians should improve their skills in communication with lung cancer patients/relatives and become more familiar with this multidimensional assessment, repeatedly screening patients for palliative care needs. Therefore, they should receive education and training to develop palliative care knowledge, skills and attitudes. This review is dedicated to lung cancer palliative care essentials that should be within the competences of treating physicians, i.e. pneumologists/thoracic oncologists. Full Article

Strength: The Van der Linde Gang Travel back to a time before former-outlaw John Marston got pulled from his farmer life in the first title, to when he was… well, an outlaw. While you might play as Arthur Morgan, in this game you become part of John’s old gang – the Van der Linde gang – together with Marston himself and other familiar and unfamiliar characters. While RDR2 also boasts a few other strong characters, this gang is at the heart of the game and, luckily, is also its strongest element, with plenty of faces and personalities that (almost) all get their time to shine. What adds all the more to the connection and believability of these characters is the living nature of the camp itself; it's your home base outside of missions, where the gang will go about their daily tasks and have conversations & arguments that you can occasionally join in on – there are even celebrations and songs around the campfire. Even if it’s mostly just flavor, the strength of this particular feature and what it adds in terms of immersion shouldn’t be understated. With 65.5 million PlayStation 5 consoles shipped through the end of June that means 3.8 million units were shipped from July to September. This is down 1.1 million (-22.45%) from the same quarter in 2023 when 4.9 million units were shipped. For reference, the PlayStation 4 had shipped 4.2 million units in the same quarter for a lifetime total of 67.7 million units shipped as of September 30, 2017. This puts the PS5 behind shipped PS4 units by 2.2 million units. There were a total of 77.7 million games sold on the PlayStation 5 and PlayStation 4 for the quarter. This is up 10.1 million from 67.6 million during the same period a year earlier. Digital sales accounted for 70 percent of software sales. There were 5.3 million first-party games sold across the PlayStation 5 and PlayStation 4. This is up 0.6 million from 4.7 million a year ago. There were 116 million monthly active users on the PlayStation Network, an increase of nine million from 107 million a year ago. Sony did not disclose the number of PlayStation Plus subscribers. It was at 47.4 million at the end of March 2023. Sony's Game & Network Services Segment for the quarter ending September 30, 2024, reported revenue increased 244.5 billion yen ($1.60 billion) year-over-year to 2,973.4 billion yen ($19.46 billion), while operating income increased 142.0 billion yen ($0.93 billion) to 389.3 billion yen ($2.55 billion). At the time Nintendo did not share which patents Palworld infringed on, however, Pocketpair has now revealed what patents Nintendo are claiming were infringed on. "As announced on September 19, 2024, The Pokémon Company and Nintendo Co Ltd (hereinafter referred to as the Plaintiffs) have filed a patent infringement lawsuit against us," said Pocketpair. "We have received inquiries from various media outlets regarding the status of the lawsuit, and we would like to report the details and current status of this case as follows. "The Plaintiffs claim that Palworld, released by us on January 19, 2024, infringes upon the following three patents held by the Plaintiffs, and are seeking an injunction against the game and compensation for a portion of the damages incurred between the date of registration of the patents and the date of filing of this lawsuit." Nintendo and The Pokémon Company are each seeking an injunction and damages of five million yen plus late payment damages. Pocketpair added, "We will continue to assert our position in this case through future legal proceedings. Zelnick added, "Our labels are really good at that. I’m not really worried. I’ve never worried about where hardware was going. And I’ve said this many times over the years because, first of all, I don’t worry about things over which I have zero control. "Secondly, I believe in the audience. The audience is going to show up if you have great properties, and so we just have to make sure to be on an array of platforms." Zelnick added PC is an important platform to release games on, despite the fact Grand Theft Auto VI has not been announced for PC. "If one platform diminishes in value, there’s always another one," he said. "So we’re seeing great growth in PC right now, for example. And I have been able to view that open formats would continue to grow. PC is an open format. "Currently, we are still in the process of learning," said Totoki (via VideoGamesChronicle). "And basically, with regards to new IP, of course, you don’t know the result until you actually try it. "So for us, for our reflection, we probably need to have a lot of gates, including user testing or internal evaluation, and the timing of such gates. And then we need to bring them forward, and we should have done those gates much earlier than we did. "Also, we have a siloed organisation, so going beyond the boundaries of those organisations in terms of development, and also sales, I think that could have been much smoother. "And then going forward, in our own titles and in third-party titles, we do have many different windows. And we want to be able to select the right and optimal window so that we can deploy them on our own platform without cannibalization, so that we can maximize our performance in terms of title launches. That’s all I have." Sony senior vice president for finance and IR Sadahiko Hayakawa discussed the success of Helldivers 2 and the failure of Concord. "We launched two live-service games this year," he said. "Helldivers 2 was a huge hit, while Concord ended up being shut down. We gained a lot of experience and learned a lot from both. "We intend to share the lessons learned from our successes and failures across our studios, including in the areas of title development management as well as the process of continually adding expanded content and scaling the service after its release so as to strengthen our development management system. "We intend to build on an optimum title portfolio during the current mid-range plan period that combines single-player games – which are our strengths and which have a higher predictability of becoming hits due to our proven IP – with live-service games that pursue upside while taking on a certain amount of risk upon release." entertainment Card-en-Ciel (NS) By www.vgchartz.com Published On :: Sun, 10 Nov 2024 17:39:16 GMT Inti Creates has made a living creating 2D side-scrolling action-platformers. So when it officially revealed Card-en-Ciel, a dungeon-diving, roguish card-collector that draws its inspiration from games like Slay the Spire and Mega Man Battle Network, it turned some heads. Indeed, this latest production represents a fairly significant departure for the independent developer, even as it embraces a familiar philosophy when it comes to art direction, music, and character design. The question remains: is this the start of something special for Inti Creates or still a work in progress? The story in Card-en-Ciel takes place in the not-too-distant future, where citizens increasingly turn to "full-dive" games, fully immersive virtual reality experiences. As these games became more popular and ubiquitous, criminal activity within the virtual space exploded. This activity, referred to collectively as "full-dive crimes", became such a problem that ordinary citizens took it upon themselves to fight back. One such crusader is Neon Nanashiki, the self-described Gaming Chair Detective. When Neon receives a suspicious email with the subject line "Please save the world", he logs into the virtual space, which turns out to be the back-end of the full-dive game Rust Tactics. There he meets a strange girl named Ancie, who explains that data from some game worlds is leaking into others, creating monsters and dungeons. Soon, Neon and Ancie team up to resolve the errors and anomalies. Despite a very promising premise, Card-en-Ciel doesn't do enough with its story. Ultimately, it's difficult to get invested in the stakes or the characters. Neon is your boilerplate archetypal hero, Ancie is frustratingly submissive, and the mystery villain generic and forgettable. There's also a some fan service here, which feels out of place. To be fair, there are some amusing conversations inside dungeons where Neon and Ancie talk about the type of game they're exploring, leading to several self-deprecating meta jokes at Inti Creates' expense, but for the most part there isn't a lot to latch on to. It hurts also that the studio dropped the ball in terms of visual storytelling. You select dungeons from a menu list, instead of traveling to them physically, and each dungeon — despite representing a wildly different game and genre — has roughly the same layout and look. It's strange that dungeons are so alike, because every one of the 300 collectible cards is entirely unique, with its own statistics, portrait art, and voice sample. As a result, collecting and experimenting with different cards is quite fun. Clearly, a lot of time, energy, and attention-to-detail went into the audiovisual design of each and every card. The same can be said for the card-battling mechanics. Inti Creates obviously took the time to sort out the various rules and synergies of each card type, allowing for many different approaches and strategies. At the simplest level, each card can be used to produce an effect or for movement along each battle map's 6x3 grid. The effects fall into three general categories of break, damage, and support. Using a card with a break effect will reduce a monster's attack power and chip away at its balance. If you reduce the enemy's balance to zero, it will enter breakdown, rendering it unable to act for one turn and opening it up to double the output from damage cards. In addition, triggering a breakdown will recover your cost by one. You only have three cost (action points) at the start of each turn, and each card has an individual cost between zero and three. Things get even more interesting with the introduction of muse cards and special skills. Once you recruit a muse card, you can activate it by fulfilling certain conditions, for example by forging three zero-cost cards. The muse will then begin their performance, bestowing unique effects during battle. Not only that, but the performance will award Neon Special Skill energy, which allows him to convert any card to a significantly more powerful variant for a turn. Consequently, in each dungeon it's important to unlock muses and then recruit cards likely to trigger the muse and/or reap the rewards of a performance. When you do this right, and lay waste efficiently to the monsters and bosses in a dungeon, it feels amazing. The problem, as mentioned before, is that each dungeon feels like the exact same thing. Apart from a specific final boss, who represents the protagonist of the fictional game world, almost everything is identical. Neon crosses the same generic square rooms & bridges and fights the same monsters over and over. It just gets boring after a while. Inti Creates really should have designed each dungeon with a layout and aesthetic unique to the IP on display. Then there's the issue with Grand Battle dungeons. These are climactic dungeons at the end of each chapter where you face off against the mystery villain. Regrettably, the rules here are different. No cards and muses can be obtained in these areas; rather you bring with you every card and muse you've accumulated to that point. The problem here is threefold: one, it removes the rogue-like flavor of standard dungeons, where you need to build a powerful deck from square one; two, it creates a lot of bloat in your deck; and three, it opens up the opportunity for irksome chain reactions, where a single card might trigger a half dozen muse performances one after the other. Card-en-Ciel partially makes up for some samey, tiresome dungeons with a huge amount of replay value. Indeed, the game is incredibly generous when it comes to content and replayability. Not only are there 300+ cards to collect, but there are 12 total main dungeons, each of which can be experienced at 11(!) different difficulty levels. And once you beat the game, you'll begin to unlock four secret dungeons. Moreover, there are daily & weekly dungeons, plus online network battles where you can fight opponents from around the world using a randomly generated starting deck. If you're someone who loves the core gameplay, you could spend dozens of hours with Card-en-Ciel. If you're more of a one-and-done sort of player, expect roughly 15 hours. Note: Inti Creates announced last week that the game will receive new content, including post-game dungeons, each month for at least five months starting this November. For a new IP, Card-en-Ciel shows potential. The premise is intriguing, the core rules and mechanics provide a strong foundation for tactical decision-making, and the replay value is absurdly high. Regrettably, the campaign that houses everything is a slight letdown, due to clichéd storytelling, forgettable characters, and generic, tedious dungeons. By retaining the core card-collecting system and enhancing the narrative and structural pieces, Inti Creates could have something special on its hands. entertainment Metaphor Re:Fantazio (PS5) By www.vgchartz.com Published On :: Mon, 11 Nov 2024 14:15:24 GMT When I was reviewing Final Fantasy VII Rebirth a few months ago, it looked set to be my Game of the Year. Metaphor: ReFantazio was still a distant concept and, I thought, communication of that concept was anything great. It turns out that I was really underestimating Atlus at the time. It's been eight years since Persona 5, and maybe I had begun to forget its magic, because as a JRPG from the creators of Persona 5, the magic is undoubtedly there in Metaphor: ReFantazio. When I was younger, I used to watch the Columbo series, where you know the culprit's identity right away at the start. Metaphor: ReFantazio is a bit like that, because the introduction shows the King of Euchronia (Metaphor's fantasy world) being murdered in his sleep by an ambitious aristocrat named Louis, which throws the whole country into turmoil. From there, the protagonist and the comrades he meets all have different reasons for sharing the same goal: to prevent the wicked Louis from seizing the throne. As simple as it seems, the narrative has its fair share of surprises. This has to do with the tremendous wealth of characters in the game. Due to a certain magic, the race for the throne is turned into an election, in which the protagonists gets to runs. Adding to the eight playable characters, you meet several competitors and followers who back you. All of them have personal stories and strange personalities that make Metaphor's adventure strikingly vivid. The sub-stories are quite interesting for most part, for they convey a range of different emotions: sometime are heart-breaking, sometime heart-warming or just purely fun, and they'll keep you engaged for hours. They often leave you in suspense, since the structure of the game has you experiencing them little by little. And, just like in Persona, you can spend extensive time with your allies in a whole lot of mini sequences that play a great part in making the cast truly unforgettable. But that doesn't mean you'll forget about the main story. Metaphor: ReFantazio offers some fascinating chapters. Take for instance the Opera house one, it's a breathtaking chapter very reminiscent of the great Final Fantasy VI, absolutely loaded with memorable sequences, grandiose fights, and striking reveals. And in the end, the whole game is like that. The thirst for more is real, as Atlus always drops a little something to make you eager to progress and feel engaged - a new character, intense combat, a shocking truth, stunning landscapes, etc. Even the most common NPCs sometimes tell a tale that blows your mind. Metaphor: ReFantazio has so much to offer in terms of characters, creativity, and storytelling that I couldn't help feeling overwhelmed at times. The incongruity of an election in a kingdom ruled by Royals for ages is in reality an occasion for the story to develop elaborate thoughts about democracy and society. The protagonist always carries with him a strange book about an ideal world, where people have equal chances, decide their own fates, and live in harmony. It's a dream-like realm wholly different from Euchronia, which is a medieval regime plagued by injustice, greed, and poverty. But there are whispers of a past, of a very advanced civilisation that ultimately failed, creating a scenario that raises the question of the relationship between democracy and freedom. There are also different tribes in the game, and it's worth noticing that the protagonist's party has one representative of each tribe, like a call for tolerance in a kingdom known for its inequality. The narrative therefore has powerful philosophic elements - something that only the greatest works in JRPG history possess. Metaphor: ReFantazio boasts world design like you seldom see, starting with the giant flying rock that has symbolized it since reveal. Here again Atlus' creativity is absolutely inspired. The designers left their comfort zone and came up with an innovative take on characters, meaning that they ditched the classical human hero or heroine. You befriend an elf-like girl, a bat-like character or a maiden with a third eye, but no real typical human. The result is a party that literally breathes fantasy, a bit like Final Fantasy Tactics Advance did in its time. Humans exist, though, but on the opposite side of the divide. These so-called Humans are twisted and awfully misshapen creatures roaming Euchronia as dangerous enemies. Atlus went beyond the boundaries of the bizarre with these designs and the result is amazing. I could also mention the funny Runner, a land vessel with legs that the party owns. Just travelling with it feels incredible. There are numerous examples like this, but in short the world is packed with wonders that I've never seen in a game before. As for combat, Metaphor: ReFantazio is a good old turn-based JRPG, where strategy and careful planning matter. Characters wield the power of Archetypes, abstract figures that party members turn into when attacking. Archetypes are essentially jobs, hence my mention of Final Fantasy Tactics above. You can equip a character with an archetype, and they then acquire attack and passive skills specific to that archetype. For example, the Gunner can deal what is called "pierce" damage from behind, the Warrior heavy "slash" damage with its greatsword, and the Mage masters the natural elements. There are also very complex Archetypes like the Masked Dancer, which can wear different masks to mimic other Archetypes, or the all-powerful Summoner, who masters absolutely ever type of damage once you acquire the corresponding stones (needless to say, these are incredibly difficult to come by). Unlike Persona 5, every character can equip any archetype, so you can choose the allies you like the most. Besides that, the currently-equipped Archetype can also inherit a couple of skills or spells from another, allowing for very elaborate lineups. Combat isn't simplistic or easy; basic skills won't do much against bosses or even the relatively big enemies within dungeons. For these, the player must analyze how Archetypes can cooperate. Two Healers can, for example, cast a light spell on all opponents, while a single healer only targets one. Three magic-type archetypes lets the Mage unleash the mightiest fire spell available, which comes in handy at times. Such cooperation skills take more than one turn crystal (as turns are represented in game), but you can save turn crystals by exploiting the enemy's weakness, like you can in Shin Megami Tensei and its spin-offs. The possibilities offered by Archetypes are fantastic and their complexity goes well beyond anything I've experienced from Atlus thus far. You have several types of Archetypes that work against a set weakness (three Archetype lineages have fire spells, for example). So you can choose the ones that fit your current goal or preferences. Boss fights are no joke and it's a renewed pleasure to overcome the various (tough) challenges after devising the right strategy. Metaphor: ReFantazio also lives up to Persona 5 in two particular aspects: main dungeons are never short of surprises, and the presentation of the battle menu is once again fantastic. Like in Persona, Metaphor: ReFantazio is played in limited time. There's an in-game calendar with deadlines set to clear the main dungeons. So you can't have everyone master every archetype - you need to choose and use every day wisely. Unlike Persona 5, which gives you fixed Personas that you have to figure out how to use optimally, building characters and Archetypes in Metaphor: ReFantazio must be planned well ahead of time. This a fundamental difference between the two and the reason why this new IP elevates Atlus' core gameplay conceits, which were already very good. Exploration is important and quite entertaining too: you can tackle tricky secondary dungeons, discover special shops, or visit your followers. The latter provide perks, like increased slots for inherited skills, greater experience gain and, most importantly, the ability to unlock advanced Archetypes. The Healer can become the Cleric, for instance, with far superior healing capabilities, and you can become a General after being a Commander, gaining powerful ice skills in addition to the potent fire skills the latter had. The whole game must be seen as a wide array of possibilities, with the player finding their own way to victory with their choices and favorite characters. As far as I'm concerned, this feels rewarding beyond anything I expected. Overflowing with content, the game is very long (around 70 hours), even with Atlus allowing you save time (any low-level enemy on the map can be beaten instantly without entering a turn-based battle, and you'll still gain experience and materials). The visuals are Metaphor's clear weakness. The game modeling isn't what you'd expect from a PS5 game in 2024, and I'd say it's fairly underwhelming for PS4 too. The backgrounds aren't very sharp and you see aliasing here and there. Secondary dungeons look very common; actually, they don't look like much of anything really. And there are several towers to explore across Euchronia, but the interior of each tower looks the exact same everytime. This kind of reuse of assets is fairly disappointing for a game of this magnitude. I shiver at the thought of what Metaphor: ReFantazio could have looked like with a Final Fantasy-sized budget. I expect it would have been my first perfect score, but it looks like I'll need to withhold that one once again. One aspect that does score perfect marks is the original music score. Here again, I was pretty sure that Final Fantasy VII Rebirth would be my soundtrack of the year, but Metaphor's music blows it away completely. Atlus' sound team has crafted vibrant choruses befitting the heroic fantasy genre, but with such unorthodox compositions that it felt fresh to my ears. Other vocals and melodies are bewitching to the point they lift your spirit in battle or dungeons. And there's the strange theme of Virga island, with its weird vocals and hypnotic murmurs. It sounds like a tribal chant from afar, a bit like what Genshin Impact achieved with the Natlan soundtrack two months ago. The music is a splendid journey in itself. Metaphor: ReFantazio is the type of game you feel was made to achieve something greater than just sales. It's a completely new world, boasting an immense wealth of gameplay, characters, locations, and music, redefining heroic fantasy like no JRPG has done for decades. Atlus' latest title is also a fantastic challenge for turn-based combat and strategy lovers, with highly enjoyable and rewarding dungeons. Put another way, Metaphor: ReFantazio is the golden age of fantasy JRPGs, revived from the ashes in all its majesty, no less. During the day, Thomas is a normal account manager.
