Bronchiectasis is a chronic lung condition which is characterised by recurrent chest infections, chronic sputum production and cough, and limited exercise tolerance. While bronchiectasis may be caused by various aetiologies, these features are shared by most patients with bronchiectasis regardless of the cause. This review consolidates the existing evidence on patient-managed interventions for adults with bronchiectasis, while also outlining areas for future research. Airway clearance techniques and hyperosmolar agents are key components of the bronchiectasis management and consistently recommended for clinical implementation. Questions around their prescription, such as optimal sequence of delivery, are still to be answered. Pulmonary rehabilitation and exercise are also recommended for patients with bronchiectasis. Relatively strong evidence underpins this recommendation during a clinically stable stage of the disease, although the role of pulmonary rehabilitation following an exacerbation is still unclear. Additionally, self-management programmes feature prominently in bronchiectasis treatment, yet the lack of consensus regarding their definition and outcomes presents hurdles to establishing a cohesive evidence base. Moreover, cough, a cardinal symptom of bronchiectasis, warrants closer examination. Although managing cough in bronchiectasis may initially appear risky, further research is necessary to ascertain whether strategies employed in other respiratory conditions can be safely and effectively adapted to bronchiectasis, particularly through identifying patient responder populations and criteria where cough may not enhance airway clearance efficacy and its control is needed. Overall, there is a growing recognition of the importance of patient-managed interventions in the bronchiectasis management. Efforts to improve research methodologies and increase research funding are needed to further advance our understanding of these interventions, and their role in optimising patient care and outcomes.

BACKGROUND:Practice on fasting prior to extubation in critically ill patients is variable. Efficacy of fasting in reducing gastric volume has not been well established. The primary objective of this study was to assess the effect of 4 h of fasting on prevalence of empty stomach using gastric ultrasonography in critically ill subjects who are fasted for extubation. The secondary objectives were to evaluate the change in gastric volumes during 4 h of fasting and to determine factors associated with empty stomach after fasting.METHODS:This was a single-center, prospective, observational study on adult ICU subjects who were enterally fed for at least 6 h continuously and mechanically ventilated. Gastric ultrasound was performed immediately prior to commencement of fasting, after 4 h of fasting, and after nasogastric (NG) aspiration after 4 h of fasting. An empty stomach was defined as a gastric volume ≤ 1.5 mL/kg.RESULTS:Forty subjects were recruited, and 38 (95%) had images suitable for analysis. The prevalence of empty stomach increased after 4 h of fasting (25 [65.8%] vs 31 [81.6%], P = .041) and after 4 h of fasting with NG aspiration (25 [65.8%] vs 34 [89.5%], P = .008). There was a significant difference in median (interquartile range) gastric volume per body weight between before fasting and 4 h after fasting (1.0 [0.5–1.8] mL/kg vs 0.4 [0.2–1.0] mL/kg, P < .001). No patient factors were associated with higher prevalence of empty stomach after 4 h of fasting.CONCLUSIONS:Most mechanically ventilated subjects had empty stomachs prior to fasting for extubation. Fasting for 4 h further increased the prevalence of empty stomach at extubation to > 80%.

Purpose Low-income children experience disproportionately high rates of dental caries and challenges in accessing dental care compared to their higher-income peers. The purpose of this scoping review was to examine the prevalence of dental caries and dental service utilization among Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) enrolled children.Methods The literature search and review were conducted between September 2023 and February 2024. The review followed the PRISMA-ScR reporting guidelines and included three databases: PubMed, CINAHL, and Dentistry & Oral Sciences Source. The study focused on children aged one to five participating in WIC within the United States (US) and aimed to determine the prevalence of dental service utilization and dental caries in the targeted population.Results This review includes twelve articles that are quantitative observational studies conducted from February 2001 to February 2023. Most of the studies were conducted in WIC programs in the Southern and Midwest regions of the US. Dental caries rates decreased by 61.8% from 2004 to 2016, with the highest prevalence in 2004, and the lowest prevalence in 2016. Dental service utilization among WIC children increased by 56.9% from 1992 to 2020.Conclusion There has been an increase in dental service utilization among WIC-enrolled children, with an overall decrease in dental caries over the last two decades. However, the prevalence of dental caries remains disproportionately high for children enrolled in WIC when compared to non-participants. To develop effective dental interventions for children enrolled in WIC, it is fundamental to identify the unique determinants of dental caries in this population.

Bacterial regulatory RNAs (sRNAs) are important players to control gene expression. In Staphylococcus aureus, SprC is an antivirulent trans-acting sRNA known to base-pair with the major autolysin atl mRNA, preventing its translation. Using MS2-affinity purification coupled with RNA sequencing, we looked for its sRNA-RNA interactome and identified 14 novel mRNA targets. In vitro biochemical investigations revealed that SprC binds two of them, czrB and deoD, and uses a single accessible region to regulate its targets, including Atl translation. Unlike Atl regulation, the characterization of the SprC-czrB interaction pinpointed a destabilization of the czrAB cotranscript, leading to a decrease of the mRNA level that impaired CzrB zinc efflux pump expression. On a physiological standpoint, we showed that SprC expression is detrimental to combat against zinc toxicity. In addition, phagocyctosis assays revealed a significant, but moderate, increase of czrB mRNA levels in a sprC-deleted mutant, indicating a functional link between SprC and czrB upon internalization in macrophages, and suggesting a role in resistance to both oxidative and zinc bursts. Altogether, our data uncover a novel pathway in which SprC is implicated, highlighting the multiple strategies used by S. aureus to balance virulence using an RNA regulator.

Endocannabinoids, which are present throughout the central nervous system (CNS), can activate cannabinoid receptors 1 and 2 (CB1 and CB2). CB1 and CB2 agonists exhibit broad anti-inflammatory properties, suggesting their potential to treat inflammatory diseases. However, careful evaluation of abuse potential is necessary. This study evaluated the abuse potential of lenabasum, a selective CB2 receptor agonist in participants (n = 56) endorsing recreational cannabis use. Three doses of lenabasum (20, 60, and 120 mg) were compared with placebo and nabilone (3 and 6 mg). The primary endpoint was the peak effect (E_max) on a bipolar Drug Liking visual analog scale (VAS). Secondary VAS and pharmacokinetic (PK) endpoints and adverse events were assessed. Lenabasum was safe and well tolerated. Compared with placebo, a 20-mg dose of lenabasum did not increase ratings of Drug Liking and had no distinguishable effect on other VAS endpoints. Dose-dependent increases in ratings of Drug Liking were observed with 60 and 120 mg lenabasum. Drug Liking and all other VAS outcomes were greatest for nabilone 3 mg and 6 mg, a medication currently approved by the US Food and Drug Administration (FDA). At a target therapeutic dose (20 mg), lenabasum did not elicit subjective ratings of Drug Liking. However, supratherapeutic doses of lenabasum (60 and 120 mg) did elicit subjective ratings of Drug Liking compared with placebo. Although both doses of lenabasum were associated with lower ratings of Drug Liking compared with 3 mg and 6 mg nabilone, lenabasum does have abuse potential and should be used cautiously in clinical settings.

⁹-Tetrahydrocannabinol (THC) is a psychoactive phytocannabinoid found in the Cannabis sativa plant. THC is primarily metabolized into 11-hydroxy-⁹-tetrahydrocannabinol (11-OH-THC) and 11-nor-9-carboxy-⁹-tetrahydrocannabinol (COOH-THC), which may themselves be psychoactive. There is very little research-based evidence concerning the pharmacokinetics and pharmacodynamics of 11-OH-THC as an individual compound. Male C57BL/6 mice were treated with THC or 11-OH-THC via intraperitoneal injection, tail vein intravenous injection, or oral gavage, and whole-blood compound levels were measured to determine pharmacokinetic parameters [C_max, time to C_max (T_max), elimination half-life, area under the curve, apparent volume of distribution, systemic clearance, terminal rate constant, and absolute bioavailability] while also monitoring changes in catalepsy, body temperature, and nociception. 11-OH-THC achieved a T_max at 30 minutes for all routes of administration. The maximum concentration at 30 minutes was not different between intravenous and intraperitoneal routes, but the oral gavage C_max was significantly lower. THC had a 10-minute time to the maximum concentration, which was the first blood collection time point, for intravenous and intraperitoneal and 60 minutes for oral gavage, with a lower C_max for intraperitoneal and oral gavage compared with intravenous. When accounting for circulating compound levels and ED₅₀ responses, these data suggest that 11-OH-THC was 153% as active as THC in the tail-flick test of nociception and 78% as active as THC for catalepsy. Therefore, 11-OH-THC displayed equal or greater activity than the parent compound THC, even when accounting for pharmacokinetic differences. Thus, the THC metabolite 11-OH-THC likely plays a critical role in the bioactivity of cannabis; understanding its activity when administered directly will aid in the interpretation of future animal and human studies.

BACKGROUND AND PURPOSE:

A cleftlike nonenhancing hypointensity was observed repeatedly in the pituitary gland at the adenohypophysis/neurohypophysis border on contrast-enhanced 3D fat-saturated T1-MPRAGE using clinical 7T MRI. Our primary goal was to assess the prevalence of this finding. The secondary goals were to evaluate the frequency of other incidental pituitary lesions, MRI artifacts, and their effect on pituitary imaging on the contrast-enhanced 3D fat-saturated T1 MPRAGE at 7T.

It appears Donald Trump is once again attempting to silence Stormy Daniels, despite his recent convictions in that category.

MSNBC’s Rachel Maddow reports that Trump’s lawyers tried to “get another hush money deal” with the adult film star, to keep her from making any “public or private statements related to any alleged past interactions” with the former president. In exchange for her written agreement, Trump’s team reportedly offered to adjust the debt she owes Trump for the unsuccessful defamation case her lawyer brought against him in 2018.

Daniels still needs to pay “hundreds of thousands of dollars” in legal fees, Maddow explained, and in hammering out the exact amount, Trump’s lawyers allegedly offered to “pretend” she owed their client “less than they actually believed” she did. Whereas they first estimated Daniels’ debt at $650,000, Maddow reported, they said they would settle her tab for $620,000, if she promised not to make any “defamatory or disparaging statements about him, his business, and/or any affiliates, or his suitability as a candidate for president.” They then adjusted the fee, asking $635,000 if she refused to sign a non-disclosure agreement. Daniels reportedly turned them down, paying $627,500 and declining to sign the NDA.

WASHINGTON — For U.S. presidents, meeting the families of military personnel killed in war is about as wrenching as the presidency gets. President Donald Trump’s suggestion Monday that his predecessors fell short in that duty brought a visceral reaction from those who witnessed those grieving encounters.

“He’s a deranged animal,” Alyssa Mastromonaco, a former deputy chief of staff to President Barack Obama, tweeted about Trump. With an expletive, she called Trump’s statement in the Rose Garden a lie.

Trump said in a news conference he had written letters to the families of four soldiers killed in an Oct. 4 ambush in Niger and planned to call them, crediting himself with taking extra steps in honoring the dead properly. “Most of them didn’t make calls,” he said of his predecessors. He said it’s possible that Obama “did sometimes” but “other presidents did not call.”

The record is plain that presidents reached out to families of the dead and to the wounded, often with their presence as well as by letter and phone. The path to Walter Reed and other military hospitals, as well as to the Dover, Delaware, Air Force Base where the remains of fallen soldiers are often brought, is a familiar one to Obama, George W. Bush and others.

Bush, even at the height of two wars, “wrote all the families of the fallen,” said Freddy Ford, spokesman for the ex-president. Ford said Bush also called or met “hundreds, if not thousands” of family members of the war dead.

Obama’s official photographer, Pete Souza, tweeted that he photographed Obama “meeting with hundreds of wounded soldiers, and family members of those killed in action.” Others recalled his frequent visits with Gold Star families, and travels to Walter Reed, Dover and other venues with families of the dead and with the wounded.

Retired Gen. Martin E. Dempsey, the former chairman of the Joint Chiefs of Staff, confirmed these contacts, tweeting: “POTUS 43 & 44 and first ladies cared deeply, worked tirelessly for the serving, the fallen, and their families. Not politics. Sacred Trust.”

Trump addressed the matter when asked why he had not spoken about the four soldiers killed in Niger. They died when militants thought to be affiliated with the Islamic State group ambushed them while they were patrolling in unarmored trucks with Nigerien troops.

“I actually wrote letters individually to the soldiers we’re talking about, and they’re going to be going out either today or tomorrow,” he said, meaning he wrote to the families of the fallen soldiers. He did not explain why letters had not been sent yet, more than a week after the attack.

“If you look at President Obama and other presidents, most of them didn’t make calls,” Trump said.

Pressed on that statement later, he said of Obama: “I was told that he didn’t often, and a lot of presidents don’t. They write letters.” He went on: “President Obama, I think, probably did sometimes, and maybe sometimes he didn’t. I don’t know. That’s what I was told. … Some presidents didn’t do anything.”

Press Secretary Sarah Huckabee Sanders said later that Trump “wasn’t criticizing predecessors, but stating a fact.” She argued that presidents didn’t always call families of those killed in battle: “Sometimes they call, sometimes they send a letter, other times they have the opportunity to meet family members in person.”

She said anyone claiming a former president had called every family was “mistaken.”

Bush’s commitment to writing to all military families of the dead and to reaching out by phone or meeting with many others came despite the enormity of the task. In the Iraq war alone, U.S. combat deaths were highest during his presidency, exceeding 800 each year from 2004 through 2007. The number fell to 313 in Bush’s last year in office as the insurgency faded. Bush once said he felt the appropriate way to show his respect was to meet family members in private.

Obama declared an end to combat operations in Iraq in August 2010 and the last U.S. troops were withdrawn in December 2011. As Obama wound down that war, he sent tens of thousands more troops into Afghanistan in 2009 and 2010, and the death count mounted. From a total of 155 Americans killed in Afghanistan in 2008, which was Bush’s last full year in office, the number jumped to 311 in 2009 and peaked the next year at 498. In all, more than 1,700 died in Afghanistan on Obama’s watch.

Among other rituals honoring military families, the Obamas had a “Gold Star” Christmas tree in the White House decorated with hundreds of photos and notes from people who had lost loved ones in war. Gold Star families visited during the holidays, bringing ornaments.

Trump visited Dover early in his presidency, going in February with his daughter Ivanka for the return of the remains of a U.S. Navy SEAL killed during a raid in Yemen, William “Ryan” Owens.

Trump’s relations with Gold Star families have not always been smooth, dating from his belittlement of the parents of slain U.S. soldier Humayun Khan, who was Muslim. Trump was angered when the soldier’s father, Khizr Khan, was given a platform to criticize him at the Democratic National Convention.

Owens’ grieving father said he didn’t want to talk with Trump at Dover. But the sailor’s widow, Carryn, attended Trump’s address to Congress and wept as he thanked her.
