Prevalence of Rathke Cleft and Other Incidental Pituitary Gland Findings on Contrast-Enhanced 3D Fat-Saturated T1 MPRAGE at 7T MRI [CLINICAL PRACTICE]

BACKGROUND AND PURPOSE:

A cleftlike nonenhancing hypointensity was observed repeatedly in the pituitary gland at the adenohypophysis/neurohypophysis border on contrast-enhanced 3D fat-saturated T1-MPRAGE using clinical 7T MRI. Our primary goal was to assess the prevalence of this finding. The secondary goals were to evaluate the frequency of other incidental pituitary lesions, MRI artifacts, and their effect on pituitary imaging on the contrast-enhanced 3D fat-saturated T1 MPRAGE at 7T.

MATERIALS AND METHODS:

One hundred patients who underwent 7T neuroimaging between October 27, 2021, and August 10, 2023, were included. Each case was evaluated for cleftlike pituitary hypointensity, pituitary masses, and artifacts on contrast-enhanced 3D fat-saturated T1 MPRAGE. Follow-up examinations were evaluated if present. The average prevalence for each finding was calculated, as were descriptive statistics for age and sex.

RESULTS:

A cleftlike hypointensity was present in 66% of 7T MRIs. There were no significant differences between the "cleftlike present" and "cleftlike absent" groups regarding sex (P = .39) and age (P = .32). The cleftlike hypointensity was demonstrated on follow-up MRIs in 3/3 patients with 7T, 1/12 with 3T, and 1/5 with 1.5T. A mass was found in 22%, while 75% had no mass and 3% were indeterminate. A mass was found in 18 (27%) of the cleftlike present and 4 (13%) of the cleftlike absent groups. The most common mass types were Rathke cleft cyst in 7 (31.8%) patients, "Rathke cleft cyst versus entrapped CSF" in 6 (27.3%), and microadenoma in 6 (22.2%) in the cleftlike present group. There were no significant differences in the mass types between the cleftlike present and cleftlike absent groups (P = .23). Susceptibility and/or motion artifacts were frequent using contrast-enhanced 3D fat-saturated T1 MPRAGE (54%). Artifact-free scans were significantly more frequent in the cleftlike present group (P = .03).

CONCLUSIONS:

A cleftlike nonenhancing hypointensity was frequently seen on the contrast-enhanced 3D fat-saturated T1 MPRAGE images at 7T MRI, which most likely represents a normal embryologic Rathke cleft remnant and cannot be seen in lower-field-strength MRIs. Susceptibility and motion artifacts are common in the sella. They may affect image quality, and the artifacts at 7T may lead to an underestimation of the prevalence of the Rathke cleft and other incidental findings.

Diffusion Analysis of Intracranial Epidermoid, Head and Neck Epidermal Inclusion Cyst, and Temporal Bone Cholesteatoma [CLINICAL PRACTICE]

BACKGROUND AND PURPOSE:

Intracranial epidermoids temporal bone cholesteatomas, and head and neck epidermal inclusion cysts are typically slow-growing, benign conditions arising from ectodermal tissue. They exhibit increased signal on DWI. While much of the imaging literature describes these lesions as showing diffusion restriction, we investigated these qualitative signal intensities and interpretations of restricted diffusion with respect to normal brain structures. This study aimed to quantitatively evaluate the ADC values and histogram features of these lesions.

MATERIALS AND METHODS:

This retrospective study included children with histologically confirmed diagnoses of intracranial epidermoids, temporal bone cholesteatomas, or head and neck epidermal inclusion cysts. Lesions were segmented, and voxelwise calculation of ADC values was performed along with histogram analysis. ADC calculations were validated with a second analysis software to ensure accuracy. Normal brain ROIs—including the cerebellum, white matter, and thalamus—served as normal comparators. Correlational analysis and Bland-Altman plots assessed agreement among software tools for ADC calculations. Differences in the distribution of values between the lesions and normal brain tissues were assessed using the Wilcoxon rank sum and Kruskal-Wallis tests.

RESULTS:

Forty-eight pathology-proved cases were included in this study. Among them, 13 (27.1%) patients had intracranial epidermoids 14 (29.2%) had head and neck epidermal inclusion cysts, and 21 (43.7%) had temporal bone cholesteatomas. The mean age was 8.67 (SD, 5.30) years, and 27 (56.3%) were female. The intraclass correlation for absolute agreement for lesional ADC between the 2 software tools was 0.997 (95% CI, 0.995–0.998). The intracranial epidermoid head and neck epidermal inclusion cyst, and temporal bone cholesteatoma median ADC values were not significantly different (973.7 versus 875.7 versus 933.2 x 10–6 mm2/s, P = .265). However, the ADCs of the 3 types of lesions were higher than those of 3 normal brain tissue types (933 versus 766, x 10–6 mm2/s, P < .001).

CONCLUSIONS:

The ADC values of intracranial epidermoids, temporal bone cholesteatomas, and head and neck epidermal inclusion cysts are higher than those of normal brain regions. It is not accurate to simply classify these lesions as exhibiting restricted diffusion or reduced diffusivity without considering the tissue used for comparison. The observed hyperintensity on DWI compared with the brain is likely attributable to a relatively higher contribution of the T2 shinethrough effect.

Predictors and Outcomes of Periprocedural Intracranial Hemorrhage after Stenting for Symptomatic Intracranial Atherosclerotic Stenosis [CLINICAL PRACTICE]

BACKGROUND AND PURPOSE:

Periprocedural intracranial hemorrhage is one of common complications after stent placement for symptomatic intracranial atherosclerotic stenosis. This study was conducted to demonstrate predictors and long-term outcomes of periprocedural intracranial hemorrhage after stent placement for symptomatic intracranial atherosclerotic stenosis.

MATERIALS AND METHODS:

We retrospectively analyzed patients with symptomatic intracranial atherosclerotic stenosis stent placement in a prospective cohort at a high-volume stroke center. Clinical, radiologic, and periprocedural characteristics and long-term outcomes were reviewed. Periprocedural intracranial hemorrhage was classified as procedure-related hemorrhage (PRH) and non-procedure-related hemorrhage (NPRH). The long-term outcomes were compared between patients with PRH and NPRH, and the predictors of NPRH were explored.

RESULTS:

Among 1849 patients, 24 (1.3%) had periprocedural intracranial hemorrhage, including PRH (4) and NPRH (20). The postprocedural 30-day mRS was 0–2 in 9 (37.5%) cases, 3–5 in 5 (20.8%) cases, and 6 in 10 (41.7%) cases. For the 14 survivors, the long-term (median of 78 months) mRS were 0–2 in 10 (76.9%) cases and 3–5 in 3 (23.1%) cases. The proportion of poor long-term outcomes (mRS ≥3) in patients with NPRH was significantly higher than those with PRH (68.4% versus 0%, P = .024). Anterior circulation (P = .002), high preprocedural stenosis rate (P < .001), and cerebral infarction within 30 days (P = .006) were independent predictors of NPRH after stent placement.

CONCLUSIONS:

Patients with NPRH had worse outcomes than those with PRH after stent placement for symptomatic ICAS. Anterior circulation, severe preprocedural stenosis, and recent infarction are independent predictors of NPRH.

Optimal Endovascular Therapy Technique for Isolated Intracranial Atherothrombotic Stroke-Related Large-Vessel Occlusion in the Acute-to-Subacute Stage [CLINICAL PRACTICE]

BACKGROUND AND PURPOSE:

Reocclusion after treatment is a concern in endovascular therapy for isolated intracranial atherothrombotic stroke-related large-vessel occlusion (AT-LVO). However, the optimal endovascular therapy technique for AT-LVO has not yet been investigated. This study evaluated the optimal endovascular therapy technique for AT-LVO in a real-world setting.

MATERIALS AND METHODS:

We conducted a historical, multicenter registry study at 51 centers that enrolled patients with AT-LVO. We divided the patients into 3 groups based on the endovascular therapy technique: mechanical thrombectomy alone, percutaneous transluminal angioplasty (PTA), and stent deployment. Mechanical thrombectomy alone was classified into the mechanical thrombectomy-only group; PTA and mechanical thrombectomy–PTA, into the PTA group; and mechanical thrombectomy–stent deployment, mechanical thrombectomy–PTA–stent deployment, PTA–stent deployment, and stent deployment–only into the stent group. The primary outcome was incidence of reocclusion of the treated vessels within 90 days of endovascular therapy completion.

RESULTS:

We enrolled 770 patients and analyzed 509 patients. The rates in the mechanical thrombectomy-only, PTA, and stent deployment groups were 40.7%, 44.4%, and 14.9%, respectively. Incidence rate of residual stenosis >70% of final angiography was significantly higher in the mechanical thrombectomy-only group than in the PTA and stent deployment groups (mechanical thrombectomy-only versus PTA versus stent deployment: 34.5% versus 26.3% versus 13.2%, P = .002). Reocclusion rate was significantly lower in the PTA group than in the mechanical thrombectomy-only group (adjusted hazard ratio, 0.48; 95% CI, 0.29–0.80). Of the patients, 83.5% experienced reocclusion within 10 days after endovascular therapy. Alarmingly, a substantial subset (approximately 62.0%) of patients experienced reocclusion within 2 days of endovascular therapy. Incidence of mRS scores of 0–2 ninety days after endovascular therapy was not significantly different among the 3 groups. Incidences of symptomatic intracranial hemorrhage, any other intracranial hemorrhage, and death were not significantly different.

CONCLUSIONS:

Incidence rate of reocclusion was significantly lower in the PTA group than in the mechanical thrombectomy-only group. We found no meaningful difference in reocclusion rates between the stent deployment and mechanical thrombectomy-only groups. In Japan, glycoprotein IIb/IIIa inhibitors are not reimbursed. Therefore, PTA might be the preferred choice for AT-LVOs due to the higher reocclusion risk with mechanical thrombectomy-only. Reocclusion was likely to occur within 10 days, particularly within 2 days post-endovascular therapy.

Intra-Aneurysmal High-Resolution 4D MR Flow Imaging for Hemodynamic Imaging Markers in Intracranial Aneurysm Instability [RESEARCH]

BACKGROUND AND PURPOSE:

Prediction of aneurysm instability is crucial to guide treatment decisions and to select appropriate patients with unruptured intracranial aneurysms (IAs) for preventive treatment. High-resolution 4D MR flow imaging and 3D quantification of aneurysm morphology could offer insights and new imaging markers for aneurysm instability. In this cross-sectional study, we aim to identify 4D MR flow imaging markers for aneurysm instability by relating hemodynamics in the aneurysm sac to 3D morphologic proxy parameters for aneurysm instability.

MATERIALS AND METHODS:

In 35 patients with 37 unruptured IAs, a 3T MRA and a 7T 4D MRI flow scan were performed. Five hemodynamic parameters—peak-systolic wall shear stress (WSSMAX) and time-averaged wall shear stress (WSSMEAN), oscillatory shear index (OSI), mean velocity, and velocity pulsatility index—were correlated to 6 3D morphology proxy parameters of aneurysm instability—major axis length, volume, surface area (all 3 size parameters), flatness, shape index, and curvedness—by Pearson correlation with 95% CI. Scatterplots of hemodynamic parameters that correlated with IA size (major axis length) were created.

RESULTS:

WSSMAX and WSSMEAN correlated negatively with all 3 size parameters (strongest for WSSMEAN with volume (r = –0.70, 95% CI –0.83 to –0.49) and OSI positively (strongest with major axis length [r = 0.87, 95% CI 0.76–0.93]). WSSMAX and WSSMEAN correlated positively with shape index (r = 0.61, 95% CI 0.36–0.78 and r = 0.49, 95% CI 0.20–0.70, respectively) and OSI negatively (r = –0.82, 95% CI –0.9 to –0.68). WSSMEAN and mean velocity correlated negatively with flatness (r = –0.35, 95% CI –0.61 to –0.029 and r = –0.33, 95% CI –0.59 to 0.007, respectively) and OSI positively (r = 0.54, 95% CI 0.26–0.74). Velocity pulsatility index did not show any statistically relevant correlation.

CONCLUSIONS:

Out of the 5 included hemodynamic parameters, WSSMAX, WSSMEAN, and OSI showed the strongest correlation with morphologic 3D proxy parameters of aneurysm instability. Future studies should assess these promising new imaging marker parameters for predicting aneurysm instability in longitudinal cohorts of patients with IA.

Artificial Intelligence Efficacy as a Function of Trainee Interpreter Proficiency: Lessons from a Randomized Controlled Trial [RESEARCH]

BACKGROUND AND PURPOSE:

Recently, artificial intelligence tools have been deployed with increasing speed in educational and clinical settings. However, the use of artificial intelligence by trainees across different levels of experience has not been well-studied. This study investigates the impact of artificial intelligence assistance on the diagnostic accuracy for intracranial hemorrhage and large-vessel occlusion by medical students and resident trainees.

MATERIALS AND METHODS:

This prospective study was conducted between March 2023 and October 2023. Medical students and resident trainees were asked to identify intracranial hemorrhage and large-vessel occlusion in 100 noncontrast head CTs and 100 head CTAs, respectively. One group received diagnostic aid simulating artificial intelligence for intracranial hemorrhage only (n = 26); the other, for large-vessel occlusion only (n = 28). Primary outcomes included accuracy, sensitivity, and specificity for intracranial hemorrhage/large-vessel occlusion detection without and with aid. Study interpretation time was a secondary outcome. Individual responses were pooled and analyzed with the t test; differences in continuous variables were assessed with ANOVA.

RESULTS:

Forty-eight participants completed the study, generating 10,779 intracranial hemorrhage or large-vessel occlusion interpretations. With diagnostic aid, medical student accuracy improved 11.0 points (P < .001) and resident trainee accuracy showed no significant change. Intracranial hemorrhage interpretation time increased with diagnostic aid for both groups (P < .001), while large-vessel occlusion interpretation time decreased for medical students (P < .001). Despite worse performance in the detection of the smallest-versus-largest hemorrhages at baseline, medical students were not more likely to accept a true-positive artificial intelligence result for these more difficult tasks. Both groups were considerably less accurate when disagreeing with the artificial intelligence or when supplied with an incorrect artificial intelligence result.

CONCLUSIONS:

This study demonstrated greater improvement in diagnostic accuracy with artificial intelligence for medical students compared with resident trainees. However, medical students were less likely than resident trainees to overrule incorrect artificial intelligence interpretations and were less accurate, even with diagnostic aid, than the artificial intelligence was by itself.

Thus, it is important to uncover the most essential information needs regarding type 2 diabetes in general practice.AimTo identify information needs related to type 2 diabetes for GPs.Design and settingSystematic review focused on literature relating to Western countries.MethodMEDLINE, Embase, PsycInfo and CINAHL were searched from inception to January 2024. Two researchers conducted the selection process, and citation searches were performed to identify any relevant articles missed by the database search. Quality appraisal was conducted with the Mixed Methods Appraisal Tool. Meaning units were coded individually, grouped into categories, and then studies were summarised within the context of these categories using narrative synthesis. An evidence map was created to highlight research gaps.ResultsThirty-nine included studies revealed eight main categories and 36 subcategories of information needs. Categories were organised into a comprehensive hierarchical model of information needs, suggesting ‘Knowledge of guidelines’ and ‘Reasons for referral’ as general information needs alongside more specific needs on ‘Medication’, ‘Management’, ‘Complications’, ‘Diagnosis’, ‘Risk factors’, and ‘Screening for diabetes’. The evidence map provides readers with the opportunity to explore the characteristics of the included studies in detail.ConclusionThis systematic review provides GPs, policymakers, and researchers with a hierarchical model of information and educational needs for GPs, and an evidence map showing gaps in the current literature. Information needs about clinical guidelines and reasons for referral to specialised care overlapped with needs for more specific information.

Lack of Knowledge of Antibiotic Risks Contributes to Primary Care Patients Expectations of Antibiotics for Common Symptoms [Research Briefs]

Patient expectations of receiving antibiotics for common symptoms can trigger unnecessary use. We conducted a survey (n = 564) between January 2020 to June 2021 in public and private primary care clinics in Texas to study the prevalence and predictors of patients’ antibiotic expectations for common symptoms/illnesses. We surveyed Black patients (33%) and Hispanic/Latine patients (47%), and over 93% expected to receive an antibiotic for at least 1 of the 5 pre-defined symptoms/illnesses. Public clinic patients were nearly twice as likely to expect antibiotics for sore throat, diarrhea, and cold/flu than private clinic patients. Lack of knowledge of potential risks of antibiotic use was associated with increased antibiotic expectations for diarrhea (odds ratio [OR] = 1.6; 95% CI, 1.1-2.4) and cold/flu symptoms (OR = 2.9; 95% CI, 2.0-4.4). Lower education and inadequate health literacy were predictors of antibiotic expectations for diarrhea. Future antibiotic stewardship interventions should tailor patient education materials to include information on antibiotic risks and guidance on appropriate antibiotic indications.

[Developmental Biology] Glial Cell Development and Function in the Zebrafish Central Nervous System

Over the past decades the zebrafish has emerged as an excellent model organism with which to study the biology of all glial cell types in nervous system development, plasticity, and regeneration. In this review, which builds on the earlier work by Lyons and Talbot in 2015, we will summarize how the relative ease to manipulate the zebrafish genome and its suitability for intravital imaging have helped understand principles of glial cell biology with a focus on oligodendrocytes, microglia, and astrocytes. We will highlight recent findings on the diverse properties and functions of these glial cell types in the central nervous system and discuss open questions and future directions of the field.

Central sleep apnoea (CSA) is characterised by recurrent episodes of airway cessation or reduction in the absence of respiratory effort. Although CSA is less common than obstructive sleep apnoea, it shares similar symptoms. CSA can be secondary to various medical conditions, high altitude and medication exposure. CSA can also emerge during obstructive sleep apnoea therapy. There are a range of treatment options and selecting the right therapy requires an understanding of the pathophysiology of CSA. Controversies in the clinical management of chronic pulmonary aspergillosis

Chronic pulmonary aspergillosis has a range of manifestations from indolent nodules to semi-invasive infection. Patients may be asymptomatic or have chronic symptoms such as cough and weight loss or present with life-threatening haemoptysis. The physician can choose from a range of available therapies including medical therapy with antifungals, minimally invasive therapy with intracavitary antifungal therapy and surgery involving open thoracotomy or video-assisted thoracoscopic surgery. The patients with the most severe forms of pulmonary infection may not be surgical candidates due to their underlying pulmonary condition. The management of haemoptysis can include tranexamic acid, bronchial artery embolisation, antifungals or surgery. There are few controlled studies to inform clinicians managing complex cases, so a multidisciplinary approach may be helpful.

I am deeply enamoured by Dragon Age: The Veilguard's intricate and ridiculous fashion design

Some extremely fresh vintage workwear that I bought for entirely practical reasons aside, I’m not exactly a fashion person. I have nobody to impress most days but my cat, and the only item of clothing she appears to have an opinion on is my Oodie, which is very comfortable for both of us and also smells like a chicken shop, which I imagine is more pleasant for her than me.

This aside, I found myself taking a whole bunch of Dragon Age: The Veilguard screenshots as I played just to capture the RPG’s various outfits. They are ridiculous. Incredibly intricate and detailed, as well as being obscenely impractical for the most part. I do not like any of them in the sense I would wear them, but I like all of them in the sense that they display artists allowed to run free like caffeinated weasels and indulge their every whim.

Twitch introduce mandatory "Politics and Sensitive Social Issues" label, just in time for the US election

Twitch have introduced a new "Politics and Sensitive Social Issues" label for streams that "focus" on topics like "elections, civic integrity, and war or military conflict". As with the streaming giant's existing labels for M-rated material, sexual themes or depictions of gambling, the idea is that viewers can filter out such streams in advance by altering their settings.

Advertisers, similarly, can "make better choices about the content they want to advertise next to" - in other words, pull their ads from a whole swathe of material if they don't want to be associated with anything controversial. Twitch's hope is that "the labels will allow advertisers to have more context to inform which types of streams they show their ads alongside, which we expect to increase brands’ confidence in running ads on Twitch, and could bring new advertisers to our service."

California wine country tries to get back to business despite wildfire destruction

JUDY WOODRUFF: Firefighters say they are making some progress battling the wildfires in Northern California. In all, the fires have consumed more than 220,000 acres, an area larger than New York City.

More than 5,700 structures have been destroyed. And at least 41 people have died, making it the deadliest wildfire in the state’s history.

The wine industry and the tourism business connected with it are trying to take stock. More than $50 billion in California’s economy comes from the wine business. And nearly 24 million people visit the region for that reason every year.

Special correspondent Joanne Jennings reports from Napa County.

JOANNE JENNINGS, Special Correspondent: The Mayacamas mountain range creates a natural barrier between Sonoma and Napa Counties. And it is here where the massive Nuns fire is posing a tough challenge for some 11,000 firefighters who are taming the blaze with aircraft and units on the ground.

CAPT. MARK BRENNERMAN, Viejas fire Department: We’re going around and making sure none of these fires that are still smoldering and smoking, we’re not going to get another big fire out of them.

JOANNE JENNINGS: Even as firefighters are battling shifting winds, owners and workers in Wine Country are trying to determine just how much damage has been done.

The tony Highlands gated community was among the first to be consumed by flames when the Atlas fire raced through this canyon, leaving several mansions in rubble. Down the hill, at the Silverado resort, charred remnants of the Safeway PGA Tour remain. The major golf event had just wrapped up last Sunday afternoon, a few hours before flames engulfed tents and grandstands, forcing spectators and athletes to evacuate.

MAN: Do you see how it burned right up to the retaining wall here?

JOANNE JENNINGS: Silverado resident Steve Messina stayed behind and shot video of fire crews containing the flames, which consumed some condos. Within minutes, flames raced three miles down Silverado Trail, home to several storied hillside vineyards.

Most wineries in the region have been spared the worst. But hundreds suffered some damage. And at least eight vineyards have been significantly damaged or destroyed.

Pierre Birebent, who has been making wines for the family-owned Signorello estate for 20 years, rushed to his winery as quickly as he could.

PIERRE BIREBENT, Signorello Estate Vineyards: I jumped right in my truck, came down, and then when I was riding down, I saw the hill all flaming.

JOANNE JENNINGS: Two vineyard workers joined him to help save the estate’s tasting room.

PIERRE BIREBENT: But the smoke was getting very thick, and the wind was very strong. And after an hour, we couldn’t breathe anymore. At the moment, I was so upset. It was rage to see that I couldn’t do anything. But it was like fighting a giant.

JOANNE JENNINGS: The tasting room, which also housed the winery’s office and a dining room, burned to the ground. But Birebent says he wants to focused on what survived.

Fortunately, he said, the fire stopped short of reaching the vineyard, the crush pad, or any of the barrels of wine stored on site; 95 percent of this year’s grapes were already picked.

But, to be on the safe side, Birebent is taking these samples to a lab to make sure the juice is not too acidic for winemaking. If the crops are OK, a staff of 25 employees will have jobs to return to.

As the fires begin to recede and the smoke clears, people here are beginning to wonder when the tourists, who fuel much of the economy, will return.

It’s a serious concern for Andrew and Jeni (ph) Schluter, who are self-employed and are raising a young family.

ANDREW SCHLUTER, Andrew’s Tours and Transportation: I do wine tours and transportation for people. And my business started to do really, really well. I was on track to have the best month ever.

JOANNE JENNINGS: Andrew just bought this new SUV, which has been idle in his driveway collecting ash. Jeni is a personal trainer and has family who lost their homes in the fires. She’s just not sure how they’re going to make ends meet.

WOMAN: I think we’re just overwhelmed, you know? And uncertainty is kind of scary.

ANDREW SCHLUTER: We will hopefully get by for awhile, but we might make — have to make some hard decisions shortly.

JOANNE JENNINGS: While fires burn nearby, some vineyards are already open to tourists. At the Raymond Vineyard, workers are crushing grapes at a feverish pitch. The tasting room is open for the first time since the fires started.

Jeremy and Erika Moore arrived from Tennessee yesterday. They considered canceling their trip, but decided the best way they could help people here is to give them their business.

JEREMY MOORE, Tourist: On the one hand, a few hundred yards from here, you can see them shuttling up with the helicopters fighting fires, but then here it’s beautiful. They are doing some great tastings, and they are working outside on the crops. So, it’s a weird combination of tragedy, but then at the same time business must go on, too.

JOANNE JENNINGS: Proprietor Jean-Charles Boisset owns several wineries in California, France and Canada, but like many other people here, he and his family had to evacuate their home when the flames came dangerously close.

Still, he is bullish about the future of the wine industry in this region.

JEAN-CHARLES BOISSET, Boisset Collection: Napa has been one of the most amazing agricultural places in California for a long time, so it will survive those fires. What I love, as a Frenchman here in California, is that amazing American positive attitude.

We will recover. We will walk again, run again, and we will welcome all our guests and give them the dreams of fine wine.

Twitter chat: How the gun control debate mirrors larger issues of partisanship in America

What would it take to turn Texas, a Republican stronghold, into a blue state? According to data from SurveyMonkey, just remove all the gun owners from the Lone Star State and it would have gone to Hillary Clinton in 2016. You can do the same thing in liberal California. Remove all the non-gun owners and the state would have voted for Donald Trump.

That’s how divisive the issue of gun control is in American politics.

SurveyMonkey found that no other demographic — not race, religion or gender — so perfectly divided voters. In the 2016 election, 47 percent of Trump supporters said gun control was an issue important enough to influence their vote. That’s compared to just 27 percent of voters who supported Hillary Clinton.

But what does this divide mean? How is it impacting gun control policy, and how might this issue change in light of recent mass shootings like Las Vegas, Orlando and Newtown?     Touchscreens Are Out, and Tactile Controls Are Back



    Tactile controls are back in vogue. Apple added two new buttons to the iPhone 16, home appliances like stoves and washing machines are returning to knobs, and several car manufacturers are reintroducing buttons and dials to dashboards and steering wheels.

    With this “re-buttonization,” as The Wall Street Journal describes it, demand for Rachel Plotnick’s expertise has grown. Plotnick, an associate professor of cinema and media studies at Indiana University in Bloomington, is the leading expert on buttons and how people interact with them. She studies the relationship between technology and society with a focus on everyday or overlooked technologies, and wrote the 2018 book Power Button: A History of Pleasure, Panic, and the Politics of Pushing (The MIT Press). Now, companies are reaching out to her to help improve their tactile controls.

    You wrote a book a few years ago about the history of buttons. What inspired that book?

    Rachel Plotnick: Around 2009, I noticed there was a lot of discourse in the news about the death of the button. This was a couple years after the first iPhone had come out, and a lot of people were saying that, as touchscreens were becoming more popular, eventually we weren’t going to have any more physical buttons to push. This started to happen across a range of devices like the Microsoft Kinect, and after films like Minority Report had come out in the early 2000s, everyone thought we were moving to this kind of gesture or speech interface. I was fascinated by this idea that an entire interface could die, and that led me down this big wormhole, to try to understand how we came to be a society that pushed buttons everywhere we went.

    The more that I looked around, the more that I saw not only were we pressing digital buttons on social media and to order things from Amazon, but also to start our coffee makers and go up and down in elevators and operate our televisions. The pervasiveness of the button as a technology pitted against this idea of buttons disappearing seemed like such an interesting dichotomy to me. And so I wanted to understand an origin story, if I could come up with it, of where buttons came from.

    What did you find in your research?

    Plotnick: One of the biggest observations I made was that a lot of fears and fantasies around pushing buttons were the same 100 years ago as they are today. I expected to see this society that wildly transformed and used buttons in such a different way, but I saw these persistent anxieties over time about control and who gets to push the button, and also these pleasures around button pushing that we can use for advertising and to make technology simpler. That pendulum swing between fantasy and fear, pleasure and panic, and how those themes persisted over more than a century was what really interested me. I liked seeing the connections between the past and the present.

    We’ve experienced the rise of touchscreens, but now we might be seeing another shift—a renaissance in buttons and physical controls. What’s prompting the trend?

    Plotnick: There was this kind of touchscreen mania, where all of a sudden everything became a touchscreen. Your car was a touchscreen, your refrigerator was a touchscreen. Over time, people became somewhat fatigued with that. That’s not to say touchscreens aren’t a really useful interface, I think they are. But on the other hand, people seem to have a hunger for physical buttons, both because you don’t always have to look at them—you can feel your way around for them when you don’t want to directly pay attention to them—but also because they offer a greater range of tactility and feedback.

    If you look at gamers playing video games, they want to push a lot of buttons on those controls. And if you look at DJs and digital musicians, they have endless amounts of buttons and joysticks and dials to make music. There seems to be this kind of richness of the tactile experience that’s afforded by pushing buttons. They’re not perfect for every situation, but I think increasingly, we’re realizing the merit that the interface offers.

    What else is motivating the re-buttoning of consumer devices?

    Plotnick: Maybe screen fatigue. We spend all our days and nights on these devices, scrolling or constantly flipping through pages and videos, and there’s something tiring about that. The button may be a way to almost de-technologize our everyday existence, to a certain extent. That’s not to say buttons don’t work with screens very nicely—they’re often partners. But in a way, it’s taking away the priority of vision as a sense, and recognizing that a screen isn’t always the best way to interact with something.

    When I’m driving, it’s actually unsafe for my car to be operated in that way. It’s hard to generalize and say, buttons are always easy and good, and touchscreens are difficult and bad, or vice versa. Buttons tend to offer you a really limited range of possibilities in terms of what you can do. Maybe that simplicity of limiting our field of choices offers more safety in certain situations.

    It also seems like there’s an accessibility issue when prioritizing vision in device interfaces, right?

    Plotnick: The blind community had to fight for years to make touchscreens more accessible. It’s always been funny to me that we call them touchscreens. We think about them as a touch modality, but a touchscreen prioritizes the visual. Over the last few years, we’re seeing Alexa and Siri and a lot of these other voice-activated systems that are making things a little bit more auditory as a way to deal with that. But the touchscreen is oriented around visuality.

    It sounds like, in general, having multiple interface options is the best way to move forward—not that touchscreens are going to become completely passé, just like the button never actually died.

    Plotnick: I think that’s accurate. We see paradigm shifts over time with technologies, but for the most part, we often recycle old ideas. It’s striking that if we look at the 1800s, people were sending messages via telegraph about what the future would look like if we all had this dashboard of buttons at our command where we could communicate with anyone and shop for anything. And that’s essentially what our smartphones became. We still have this dashboard menu approach. I think it means carefully considering what the right interface is for each situation.

    Several companies have reached out to you to learn from your expertise. What do they want to know?

    Plotnick: I think there is a hunger out there from companies designing buttons or consumer technologies to try to understand the history of how we used to do things, how we might bring that to bear on the present, and what the future looks like with these interfaces. I’ve had a number of interesting discussions with companies, including one that manufactures push-button interfaces. I had a conversation with them about medical devices like CT machines and X-ray machines, trying to imagine the easiest way to push a button in that situation, to save people time and improve the patient encounter.

    I’ve also talked to people about what will make someone use a defibrillator or not. Even though it’s really simple to go up to these automatic machines, if you see someone going into cardiac arrest in a mall or out on the street, a lot of people are terrified to actually push the button that would get this machine started. We had a really fascinating discussion about why someone wouldn’t push a button, and what would it take to get them to feel okay about doing that.

    In all of these cases, these are design questions, but they’re also social and cultural questions. I like the idea that people who are in the humanities studying these things from a long-term perspective can also speak to engineers trying to build these devices.

    So these companies also want to know about the history of buttons?

    Plotnick: I’ve had some fascinating conversations around history. We all want to learn what mistakes not to make and what worked well in the past. There’s often this narrative of progress, that things are only getting better with technology over time. But if we look at these lessons, I think we can see that sometimes things were simpler or better in a past moment, and sometimes they were harder. Often with new technologies, we think we’re completely reinventing the wheel. But maybe these concepts existed a long time ago, and we haven’t paid attention to that. There’s a lot to be learned from the past.

    Nintendo will show off 'Donkey Kong Country' theme park expansion at live event today

    Nintendo is set to show off the Donkey Kong Country expansion of its Super Nintendo World theme park today (Monday, November 11) at 5PM ET, the company revealed in a post on X. That means the new Universal Studios Japan area might finally open soon, after being delayed from the previously announced launch window of Spring 2024. 

    The new park attraction featuring Mario's barrel-throwing nemesis was first revealed in 2021. The centerpiece is a ride called Mine Cart Madness, named after the best level in the 1994 SNES game Donkey Kong Country. After being blasted out of a barrel cannon, you'll take a mine cart ride through the jungle that includes a "leap across a collapsed track" as Nintendo showed in a render earlier this year.  

    The direct livestream appears to be focused on Donkey Kong world alone, and Nintendo specifically pointed out that "no game information will be featured" — so don't expect any news on the next-gen Switch 2 console either. 

    Donkey Kong Country will expand the park size by up to 70 percent, Nintendo said when it was announced. Current attractions at the Japan park are the Mario Kart: Kuppa's Challenge rollercoaster and Yoshi Adventure. Nintendo recently announced that it would open a Super Nintendo World attraction at Universal Studios Orlando on May 22nd, 2025 with the same attractions as the park in Osaka.

    This Shin Megami Tensei board game features 72 intricate demon figurines

    The popular Shin Megami Tensei video game series is getting a physical board game. A Japanese company called Icrea is behind the effort and is launching a Kickstarter on November 12 to raise funds for the project.

    SHIN MEGAMI TENSEI THE BOARD GAME (the company asked us to use all caps) is a “full-scale board game project” complete with 72 “high quality and detailed demon figures.” Collecting demons and adding them to your party is a major part of the game, so these figurines should help with immersion. It’ll also help with getting random demon figurines lost in the couch.

    There’s a board with various locations from the franchise and plenty of series-specific cards to rifle through. The company says a game should take anywhere from three to four hours. The title has already generated a fair amount of excitement at both Gen Con and the most recent Tokyo Game Show.

    We don’t know exactly when this will be available for purchase. You know how Kickstarter timelines work. There are going to be a bunch of stretch goals, however, should the company reach the initial financial threshold. It’s a Shin Megami Tensei board game with dozens of cool-looking demon figurines. It shouldn’t have any trouble finding financial backers.

    For the uninitiated, Shin Megami Tensei first came out in 1992 for the Super Famicom in Japan. There have been plenty of sequels throughout the years. Shin Megami Tensei V came out back in 2021, but has since received a full-featured refresh called Shin Megami Tensei V: Vengeance.

    The Persona series is also a spinoff of the franchise. This leads to a very serious question. Persona board game when? I want to wander around a fake high school and eat ramen with a motley crew of characters.

    Apple could launch a smart home control center next year

    Apple is rumored to be working on a new smart home product. Mark Gurman at Bloomberg reported that the company is developing a wall-mounted display for controlling appliances, interacting with Siri and videoconferencing. The tablet is said to look "like a square iPad" with "a roughly 6-inch screen." It would have a camera at the top as well as internal speakers and a built-in rechargeable battery. His sources said this smart home display could be officially announced as soon as March following three years in development.

    This description sounds a lot like the Google Home Hub, which came out in 2018, or the Echo Show, which is already up to its eighth iteration. If Apple wants to be a serious competitor in this smart home space, it'll have to prove to customers that it has something to offer that these more established brands and products don't.

    Right now, the only distinguishing feature is likely Apple Intelligence, which finally began rolling out to users in late October, and the upcoming App Intents system for controlling software programs with AI. But Apple may be looking farther ahead toward something more extravagant than just a smart device control panel. Gurman reported that Apple is already working on a version with a robotic arm that could move the tablet around. It should be interesting to see exactly what Apple does formally launch and when.
